Suboxone vs methadone for opioid use disorder
Suboxone is buprenorphine plus naloxone. It binds tightly to opioid receptors but only partially activates them. That partial-agonist behavior is why it has a ceiling on respiratory depression and a much lower overdose risk than methadone. It is also why it is delivered through office visits and prescriptions instead of daily clinic dosing.
Methadone is a full agonist. It is more powerful for severe long-term opioid use disorder, especially fentanyl-driven cases. The trade-off is that methadone is only legally dispensed through SAMHSA-certified opioid treatment programs, which means daily dosing visits, at least at the start.
If you are in Saint Paul weighing the two, the decision usually comes down to severity, history of treatment, and your daily logistics. Buprenorphine is easier to access. Methadone is sometimes the better clinical fit.
